To download a registration form to mail or give to the CD, click on the PDF Button below the online entry form (if available).
Sometimes you might get an error when you submit the entry form but don't worry because 99.9% of the time it will still process. Please do not resubmit if you get an error. You will get the confirmation when you call in to pay with a credit card.
